Eligibility
Eligibility is based on household income and family size, with options for discounted care for those who qualify. In addition, all veterans, active-duty military members, and first responders automatically receive discounted care in recognition of their service to our community.
If you’re unsure whether you meet the criteria, our team will gladly walk you through the application process and help you find a plan that fits your situation.
How to apply
To see if you qualify for financial assistance, simply complete our Financial Assistance Application Form. Once filled out, you can email it to our office at info@buhaywellness.org or bring it with you to your first visit. Our team will review your application and contact you to discuss your eligibility and available options.
